Severe aortic stenosis is one of the most common and serious valvular heart diseases in the elderly population, with a significant clinical impact in terms of morbidity, mortality, and quality of life. Inrecent years, the advancement of transcatheter aortic valve implantation (TAVI) techniques has revolutionized the therapeutic approach, offering an effective alternative to traditional surgery, even for patients at high or prohibitive surgical risk.

Traditionally performed via transfemoral access, TAVI is now exploring new frontiers through the diversification of vascular approaches, with the aim of expanding procedural eligibility to patients with complex anatomies or contraindications to conventional access routes. In this context, transaxillary access is emerging as a promising, safe, and reproducible option, thanks to its minimally invasive nature and the possibility of being performed under local anesthesia—resulting in reduced recovery times and perioperative complications.

This congress aims to provide a multidisciplinary, evidence-based update on the current and future role of TAVI via transaxillary access. The program will cover anatomical and technical considerations, clinical indications, patient selection criteria, as well as strategies for managing complications and ensuring long-term follow-up.
